Dear Applicant:
Thank you for your interest in the Stoddard Baptist
Nursing Home (SBNH). The following information should be submitted to the Admissions
Office for nursing home placement at SBNH.

Complete the "Referral for Long Term Care Form" (available from
the Admissions Office) page 1, identifying information. Have pages 2 and 3 of the form
completed by the applicant's physician. Return all completed forms along with the tests
and reports listed in Number 5. |

Please return all tests/reports to the Admissions Office at SBNH. |

If the applicant is eligible for Medicaid, SBNH will submit all completed
forms to:
