HEALTH INSURANCE PORTABILITY 
and ACCOUNTABILITY ACT OF 1996

 

The Federal Code of Regulations, Rule 45 C.F.R. 160 and 164 of 
the Health Insurance Portability and Accountability Act of 1996 
are requirements for Health care Providers to safeguard 
confidential health information and exempt from disclosure. 
The facility must furnish written information on the regulation.

 

The Health Insurance Portability and Accountability Act of 1996 (HIPAA), 
is a law mandated by Congress that requires health care providers to establish 
standards to ensure the protection of resident health information. 

 

Stoddard Baptist Nursing Home strives to protect and safeguard your 
health information and will not share information unless it is necessary.  
We may with your permission use and disclose medical information 
about you so that the nursing facility may receive payment for the 
services it gives you. However, it will be shared either verbally, 
in writing, or electronically to person(s) or agencies e.g. Medicare 
and Medicaid that have a need or right to the information requested, 
such as but not limited to:

 

a.        You and/or your designated point of contact

b.       Health care providers (such as your doctor, nurses, pharmacy, etc.) 
    for purposes of treatment, payment, or other health operations.

c.        Oversight agencies such as Centers for Medicare and Medicaid Services 
    (CMS), office of the Inspector General, etc.

d.       Your permission will be asked before any medical notes are used or disclosed.

 

Residents and/or Designated point of contact/Responsible Party and staff have 
the right to request restrictions on using and disclosing of health information.
